Learn more about post hospital care.

Post hospital care you can access through us is designed to care for patients and their families following a hospital discharge. We focus on a comprehensive approach that caters to the patient’s condition, promoting recovery and a return to daily life.

When to consider post hospital home care.

Our services are crucial when a patient has been discharged from the hospital and requires additional care. This is often recommended by a social worker or healthcare provider based on the patient’s condition and the expected recovery plan. It’s especially important for those who need a structured program to focus on recovery and regaining independence.

How we help people with post hospital care needs.

Following an initial assessment by our team, including your Care Manager and registered nurses, we develop a personalised Care Plan. This program is focused on the specific needs and conditions of patients, ensuring that all aspects of their recovery, from medication management to physical rehabilitation, are addressed. We aim to provide a seamless transition from hospital to home, working in tandem with family members to adapt care as the patient’s condition evolves.

Scheduling support programs at your own home.

We understand that every individual has different needs and preferences. With our self-management model, you can choose the days and times when you’d like to receive post hospital care and choose who you would like to have. Whether it’s medication management, help with daily shopping or personal care, you can always get the support you want.

How post hospital care works.

At Let’s Get Care we have the largest list of service providers in Australia which is growing every day. We don’t lock you into using our staff and make you pay inflated rates, instead we broker workers nationally. This means you have the freedom to choose and find care workers right for your situation, local to you.

Your Provider will be required to supply you with a written Care Plan. A Care Plan is designed to meet your goals and assessed care needs as determined by your ACAT assessment. Your Provider will put this Care Plan together in consultation with you during your initial meeting. It will explain in detail the needs and services of a client.

The Care Plan will set out goals and services you need to remain at home. You are required to be issued with this Care Plan within 14 days of entering into your Agreement.
