We understand how key great Care Management services are for supporting you with your Home Care Package. Even though we are a self-managed Provider, you will have a dedicated Care Manager to help guide you every step of the way.

What’s included in Care Management?

Care Management is an essential service for all Home Care Package recipients. It involves the consistent evaluation of a client’s needs, objectives, and preferences by a Care Manager. It includes the regular review of their Home Care Package and Care Plan, ensuring that the care and services provided are aligned with other available supports. The Care Management services you receive depend on the Provider you sign with and the services they offer.

Who is Care Management for?

Every Home Care Package recipient receives Care Management services through their package. Providers are required to offer these services if they are chosen to manage a recipient’s package. You decide on who you would like to offer this service and manage your Package. It’s important for you and your family to be aware that different Providers offer different services. You can discuss the degree of support you would like from a Care Manager and decide on your management option:

Our pricing.

With a flat fee of 18% with no hidden costs or additional fees, our fees are among the lowest in Australia. Lower fees help you get more out of your Home Care Package.

This provides basic support and assistance for individuals with low care needs to help them remain independent at home.
Offers a moderate level of support and services for individuals with increasing care needs, focusing on enhancing their well-being and comfort at home.
This level of funding supports clients with intermediate needs.
This is the highest level of Home Care Package support.

The first step is to speak with one of our Care Experts and we will discuss which option will best suit you and your care needs. If you have a Home Care Package we can get you started right away, if not we will help you get assessed.

Step 2

Our friendly Care Experts will arrange the signup documents for you to complete online so you can get your services started as soon as possible.

And you’re ready to get your services started! With Let’s Get Care you have the ability to choose from our huge list of amazing service suppliers or bring your own carers onboard.

We’ve put together a list of frequently asked questions to help guide you through the home care package process. We know it can be confusing, but we hope this helps to explain some of the most commonly confusing topics.

At Let’s Get Care we have the largest list of service providers in Australia which is growing every day. At Let’s Get Care we don’t lock you into using our own staff at inflated costs, instead we broker our services nationally. This means you have the freedom to choose and find carers right for your situation, local to you.

CHSP:

You’ll have access to specific supports and there isn’t much flexibility.

You pay a subsidised rate from your own pocket, for the supports you use.

 

HCP:

You get an amount of funding that you can use for whatever services you require so long as they are within government guidelines and in line with your Care Plan.

Once you have a package, it is yours until you no longer require it.

You get to choose the services you would like to use – based on your unique situation.

A Home Care Package (HCP) is funding given to older Australians through a government-subsidised program with the aim of providing long-term support for elderly individuals who want to continue living independently at home.

After your assessment, you will receive a letter from My Aged Care letting you know the outcome of your assessment. If you are eligible for support you will be approved for a specific level (1,2,3 or 4) and placed in the national queue to wait for a package to be assigned to you.

When you are assigned your package, you will receive another letter indicating your assigned level and an individual referral code. At this time, you’ll need to choose your Provider.
