After a thorough assessment, we match you with an experienced Care Manager to work with you and design your customised Care Plan. From there you get to choose the services that would suit you and your family at home. Whether that’s groups or clubs, or drawing, or painting, or other types of social support services. You will be encouraged to think about the services that would keep you most healthy, happy and safe at home.

How to set up care services.

We help you get your in-home aged care services started quickly to ensure you get the support you need with safety and continue living happily at home. With us it’s easy.

We’ve put together a list of frequently asked questions to guide you through the home care journey. We know it can be confusing, but we hope this helps to explain some of the most commonly confusing topics. 

At Let’s Get Care we have the largest list of service suppliers in Australia which is growing every day. At Let’s Get Care we don’t lock you into using our own carers at inflated costs, instead we broker our services nationally. This means you have the freedom to choose and find carers right for your situation, local to you.

This is a fantastic way of connecting with your community or a group of other seniors, creating motivation and purpose. 

You don’t need to stay with a Provider that you’re not happy with. In February 2017 Consumer Directed Care (CDC) legislation was amended which has empowered Home Care Package holders with the freedom and control to actively participate in their own care.

If you’re happy with the service you’re getting, then that’s great, but if you want to change to a better solution then you also have the option to change to other providers. 

When you decide to change Providers, check your previous Home Care Agreement for any fees or special conditions. Your Agreement will indicate your notice period to leave the Provider.

Your Provider will be required to supply you with a written Care Plan. A Care Plan is designed to meet your goals and assessed care needs as determined by your ACAT assessment. Your Provider will put this Care Plan together in consultation with you during your initial meeting. It will explain in detail the needs and services of a client.

The Care Plan will set out goals and services you need to remain at home. You are required to be issued with this Care Plan within 14 days of entering into your Home Care Agreement.

The Care Plan is a type of guide for what products and services you can organise to best support you.
